How to Request Your Health Care Records
You have the right to access your health care records maintained by our practice. To request your records, please:

Submit a written request - You may make your request in writing by email, mail, or in person during regular business hours

Provide identification - Include your full name, date of birth, and contact information to verify your identity

Specify the records needed - Indicate which records you are requesting and the date range, if applicable

Allow processing time - We will respond to your request within 15 business days as required by Texas law
